The 5-year-old’s immunization record is key for the fulfillment of the physical examination. By age 5, the boy should have received three doses of Hepatitis B, two or three doses of the Rotavirus vaccine, and five doses of Diphtheria, Tetanus, and Acellular Pertussis. Furthermore, four doses each of Haemophilus influenzae type b, Pneumococcal conjugate, and Inactivated Poliovirus vaccine should have been administered. The other vaccinations are two doses each of Varicella, Measles, Mumps, Rubella, and Hepatitis A. After checking the immunization record of the boy, I will ask the parents for any past medical history of their son. This will provide a clear picture of the recent health status of the boy.  The boy’s weight, height, Body Mass Index, temperature, pulse, respiratory rate, and blood pressure will be recorded. I will also ask about the travel history of the boy to establish the need for a Tuberculin skin test.

Since the parents are opposed to vaccinations, I will educate them on the importance of vaccines and insist on immunizing the boy. School-going children are most likely to contract diseases such as Chickenpox, Diphtheria, Tetanus, Pertussis, Measles, Mumps, Rubella, and Polio. Therefore, their son must receive the vaccinations before joining his peers. The guidelines of the Centers for Disease Control and Prevention (CDC) state that vaccinations should be administered if the immunization history is incomplete or unknown (Advisory Committee, 2017). If a vaccine was not administered at a recommended age, it should be given at a subsequent visit. The World Health Organization has similar provisions. It states that immunization should be initiated once a person that is not vaccinated visits a hospital (Larson et al., 2018). Therefore, I will immunize the boy based on the Catch-Up vaccination schedule recommended by the CDC.

The ethical principle applied in this scenario is beneficence. Beneficence is a principle that requires the action of a healthcare provider to promote good (McDermott-Levy et al., 2018). As an advanced practice nurse who uses evidence-based practice guidelines, I am obligated to protect the boy from harm. The five-year-old cannot make definitive decisions about his health. Vaccinations are important for maintaining optimal health. Therefore, even though the parents oppose the idea, the boy should be vaccinated before joining his peers in kindergarten.
