Despite the many significant advances in the treatment of pain, opioids are still the most potent analgesic medication class available. Opioids relieve most pain types, are available in almost all pharmacies, and are very safe when used appropriately. When a co-addictive disorder exists alongside pain, and more so in patients who use opioids for controlling pain, the treatment and evaluation may be challenging for physicians and caregivers while generating prolonged suffering and frustration for the patient. Similar to other contexts, all complaints a patient makes about pain ought to be evaluated carefully and thoughtfully because the pain can impair the quality of life, function, and health.

The prescription of opioids to persons with a history of substance abuse has both ethical and clinical implications. The principles of justice and beneficence assert that every person should have access to pain treatment that is effective; however, a physician’s duty to offer pain relief can conflict with the principle of non-maleficence when the use of opioids predisposes harm to the patient with SUD. When addiction and chronic pain co-occur, each may be a reinforcement of the other’s components, as illustrated in the diagram below:

The treatment of a patient with an active addiction is critical for effective chronic pain management. However, the treatment plan must accommodate the physical dependence on opioids because pain may increase when the withdrawal is initiated, which may interfere with efficacy and compliance.

Opioids, however, may be misused by individuals wanting to self-medicate for mood imbalances and, in some incidences, may result in addiction. Opioid misuse can lead to death via suppression of the respiratory system and cause cardiac arrest. In some individuals, long-term use of opioids can change the processing of pain and increase the pain levels that a person experiences (Rivat et al., 2016). Thus, a balance needs to be struck between the risks and benefits of opioid use, a challenge that calls for a basic understanding of the actions and conditions of opioids linked to their usage.
