I am trying to implement a culturally-based prevention program proposed by Kelley, Fatupaito, & Witzel (2018).

  1. What risk factors does it address?

The strategy is specifically aimed at addressing the risk of binge drinking among American Indian youth. The program was designed to mitigate the risk of substance abuse and the threat of addiction. Binge drinking, substance abuse, and addiction are interrelated and pose a major health threat to the well-being of American youth, especially Indian Americans – a minority group that is regarded as one of the highest-risk populations due to discrimination, high exposure to violence, present traumas, and several social inequities. Finally, the program was structured to lower the risk of unpreparedness, unawareness, or inexperience in the community in terms of handling substance abuse.

  1. What kind of change are you trying to implement?

The change we are trying to implement is to increase community preparedness through a cultural-based prevention program. Substance abuse is a multi-faceted problem affecting youth, adults, families, communities, schools, etc. The change we are trying to bring is to implement contextually-based and culturally-individualized programs that can be rolled out in communities and federal organizations and adopted in other parts of the globe to lower drug abuse and subsequent addiction.

The program involved one urban Indian region and five reservation communities within the Rocky Mountain Region in the United States of America. According to the 2017 report by the Billings Area Indian Health Service, nearly 8,805 of the 47,066 American Indians residing in these six regions are aged between 12 and 20.

  1. Who does the strategy benefit most? Or, to put it another way, who is your target audience?

The main beneficiary of this program is American youth, especially the minority Indian American youth between the ages of 12 and 20. Studies have shown that the rate of substance abuse is high in this population because their risk of exposure and indulgence is significantly elevated due to factors like discrimination in the country, high unemployment rate, low income, and so on. A cultural-based approach to drug abuse prevention can also benefit other populations, such as African American youths, because it targets specific traditional elements of a population.

Cultural-based prevention is a more effective approach to drug abuse prevention because it considers a community’s unique needs, resources, capacity, and culture. The method is also the most effective in evaluating a community’s readiness. The technique also integrates several factors such as family communications, self-esteem, social support, and community connections – most of which have been established to play a fundamental role in prevention and reduction of prevalence rates among drug abusers and addicts.


  1. What is the challenge that your community faces that your policy, system, or environmental change strategy addresses? 

The primary challenges the community is facing include high rates of binge drinking, increased prevalence rates of drug abuse, and substance addiction or dependence.

  1. What might happen if the challenge is not addressed? How does it affect your community in the short term? The long-term?

If the challenge is not addressed, youngsters in the state are likely to engage more in drug and substance abuse. There are short-term and long-term effects of this course. According to the National Institute on Drug Abuse (2020), short-term effects range from changes in stroke, heart rate, appetite, and blood pressure, to a heart attack. Other health effects include overdose, psychosis, stroke, and death. Long-term effects can include the development of cancer, lung disease, diabetes, kidney failure, hepatitis, HIV/AIDS, addiction, and many others.

  1. Were there any barriers encountered during the development or implementation of your policy, system, or environmental change strategy? What were they and how did you overcome them?

Selection bias is one of the main barriers encountered because of the use of homogenous purposeful sampling to collect data. Another challenge is that the respondents (youth) might have responded to the questions presented based on their social desirability. Most of these barriers were not addressed and might have affected the program’s outcomes.


  1. What are the positive results your strategy brought to the community? Try to be specific, with a few statistics or examples.

Firstly, community connections [5% level: F (1,545) =4.92] and social support [5% level: F (1,522) =15.81; p=.00] were significantly higher in the intervention group compared to the non-intervention group, according to the study done by Kelley, Fatupaito, & Witzel (2018). This implies that youngsters who participate in this program (cultural activities) have better connections to society and higher social support levels than those who do not participate. The program also increased community readiness between 2011 and 2014.
