Today, evidence-based practice (EBP) is increasingly employed in healthcare settings, especially in nursing practice, to improve quality, safety, cost of care, and other patient outcomes. The term EBP simply refers to a type of practice that is anchored on or dependent on solid research or scientific proof and sticks to established rules, codes, and standards of practice while considering the patient’s needs (Black et al., 2015). Today, several EBP initiatives are widely and increasingly used in nearly all healthcare settings, from oxygen use to treat patients with chronic obstructive pulmonary disease to patients that need an intravenous catheter.

As a nurse practitioner, I have experienced first-hand the benefits of using EBP initiatives in nursing settings. For example, in my former role as the nurse manager at The New York General Hospital (fictional name), I happened to be in charge of ensuring an EBP initiative was successfully and sustainably implemented in all the wards to ensure that patients do not contract nosocomial infections or hospital-acquired infections (HAIs). Hospital-acquired infections are usually some of the most significant safety concerns issues for most patients and care providers. They are associated with increased cost and length of hospital stay, morbidity, and mortality.

Specifically, I introduced the isolation technique as one of the general EBP strategies for controlling the infection. The first step was to assess each patient’s need for isolation, with all those individuals admitted to the ICU being screened to determine the urgency and need. The second step was to determine which isolation category was required for each patient identified, whether protective isolation or source isolation (Mehta et al., 2014). The isolation technique was successful and sustainable because we managed to reduce the rate of nosocomial infections by 50 percent within a month. We kept reducing the rate by at least 30 percent each month.

Finally, I would use Kotter & Cohen’s approach to organizational change to enhance the sustainability of the EBP initiative (patient isolation to prevent the spread of HAIs). This model is anchored on the premise that creating a sustainable organizational change requires “appealing to their emotions” (that is, assisting them to feel differently). According to Melnyk & Fineout-Overholt (2015), the following are the eight steps I would use to sustain the change. Increasing urgency, building the guiding team setting the vision correctly, communicating for ‘buy-in,’ removing barriers, taking an empowering action, coming up with short-term wins, never letting up, and making the change stick.
