It’s very informative and clearly explains what palliative care is and the ethical issues that nurses often have to deal with when caring for older patients, especially those that are terminally ill. I cannot agree more with your opinion that seniors usually live with one or more chronic illnesses, whether diabetes, arthritis, blood pressure, renal diseases, or heart disease, and thus, need extreme care. Sometimes, when faced with such persistent, end-of-life diseases, some patients tend to make their wishes clear – choosing to die at home and surrounded by their family members. As a palliative care nurse, the most appropriate thing is to engage the family in the decision-making process, which should reflect the patient’s and family’s wishes (National Institute of Aging, n.d.). This is important because nurses must respect the autonomy and independence of patients. Therefore, I think you performed exceptionally well by respecting the wish of your patient – who passed away at her home peacefully and happily.

Great post! It’s unique and very important because most people think that older and terminally-ill people, especially those staring at death imminently, are the only ones that require comfort care. As defined by the National Institute on Aging (n.d.), comfort care is a vital component of nursing care that is given to those at the end of life. It is primarily intended to soothe the dying patient and relieve the pain and distress as much as possible, as well as boost the quality of life of the individual. Most importantly, as you have stated, the care provided must be in line with the wishes of the person, meaning that nurses or healthcare providers cannot deviate from what the patient or family has chosen. However, as you have illustrated with the failure of the doctors to save the two babies, sometimes the wishes of the patient cannot be met if it is beyond the capacity of the physician or health worker.
