In many cases, injuries to the head resulting from falls, among other causes, result in head pains. However, the boy’s aunt denies such injuries, which would imply that the headaches might have other causes. Therefore, I would want to determine the causes of pain the child might have been exposed to and ascertain the source of the pain. Since the child reports no medical procedures that might have resulted in the pain, has no genetic history that can relate to the same, and reports no head injuries, he could be facing psychological and environmental influences that might result in neurological impairment or musculoskeletal pain.

Since the boy had just moved in with his aunt, her husband, and elderly cousins with whom he is finding it hard to adapt, the boy could be undergoing stress that could manifest as widespread unusual pains (Clinch & Eccleston, 2009). Additionally, his social history indicates that the school could not be a problem since he has not changed. The death of his parents could also be traumatizing. In this case, the patient requires screening for AMPS.

The priority diagnosis should be to determine if the child is suffering from Amplified Musculoskeletal Pain Syndrome (AMPS), which results in abnormal pain reflexes due to injuries, illnesses, or stress (Alriksson‐Schmidt, & Hägglund, 2016). The priority diagnosis should be for neuropathic pain and psychogenic pain. Since the painkillers given to the child are not working to improve his health, I would suggest that if he is positive for the given diagnoses, opioids should be prescribed for the patient. Besides, the patient should be referred for counseling to determine if he has challenges adapting to his new environment and how he could be assisted with PTSD from the death of his parents.
