The Scenario in Brief

Mike was running late for work once again. He had promised his supervisor that he would not be late again, after which the supervisor informed him that failure to honor his word would result in possible termination. Mike is the sole provider in his young family and does not want to risk losing his job. The next day, he leaves home 20 minutes early for work. However, there is a delay in traffic due to an accident. Additionally, when Mike gets to work, he finds a spill on the floor. After much contemplation, he opts not to wipe the spill lest he clocks in late. However, the spill causes a patient to slide and fall, resulting in a possible broken hip. The patient is devastated at the thought that the hospital did not prevent her fall.

What Were the Consequences of a Failure to Report?

Mike’s decision has negative consequences. If he had opted to report the spill, it would have resulted in his lateness and possible job termination. His failure to report the spill resulted in a patient injury, which could cause major costs to the hospital. The cost has also spilled over to the patient as she now has to be hospitalized and lose out on productivity. Additionally, the patient may seek legal redress against the hospital, which would mean the hospital’s reputation would face a level of damage. Further, the staff at the hospital will have an additional workload that could have been prevented (Spetz, Brown, & Aydin, 2015).

What impact did his decision have on patient safety, on the risk for litigation, on the organization’s quality metrics, and the workload of other hospital departments?

Mike’s decision on failure to report the spill resulted in a patient injury. Although it may be argued that it was another person’s responsibility to ensure that the floor was dry, it could also be argued that Mike had sufficient knowledge of inpatient falls in hospital settings as well. The broken hip will cause the patient to have difficulty in walking, and if she is elderly, the healing process may take a considerably long time. Hence, the patient will lose out on productivity, and eventually, the broken hip will decrease her level of mobility.

Mike’s decision exposed the healthcare facility to a possible lawsuit. The patient stated that she did not understand why the fall had to occur in the hospital, yet a hospital is supposed to be a safe place for all. Such sentiments could cause the patient to file a case against the hospital seeking payment of damages caused to her.

The hospital’s quality metrics will also be questioned following Mike’s decision. Hospitals typically have policies and outcome measures that gauge the hospital in different parameters, including service delivery, employee satisfaction, and compliance with government and regulatory measures, among others. Most of the metrics are measured against international standards, which place a hospital at a competitive advantage (Tsai et al., 2015). Therefore, when incidences occur that question the quality metrics of a hospital, the result is the loss of credibility in the industry, patient and employee dissatisfaction, and negative imagery of the brand (Tsai et al., 2015). The decision by Mike to ignore the spill will affect the good reputation of the hospital and possibly have the hospital engage in a long process of proving its credibility to the public and regulatory bodies it complies with.

The hospital departments that will be involved following Mike’s decisions and consequent patient fall include the nursing staff, radiology, pharmacology, nutritionist, and physiotherapy. All these will be involved in the treatment and healing process of the patient. Other departments that will be affected are the hospital’s litigation department, the occupational hazard department, and the cleaning services contracted by the hospital. The human resource department may be involved in the case if the litigation department decides on having the services of the contracted cleaners terminated. The legal team and occupational hazard department may need to work together in the event the patient opts to sue the company. The finance department will only be involved in the patient wins the litigation case and needs to be compensated in monetary terms.

As Mike’s manager, what will you do to address the issue with him and ensure other staff members do not repeat the same mistakes?

The scenario provides an opportunity to educate staff on the importance of putting the patient first and how doing so positively affects everyone. Failure to prioritize patients has the opposite effect. By using an evidence-based approach, such as employee engagement in patient care (Lowe, 2012), the manager is bound to yield positive outcomes. The manager should have a discussion with Mike regarding his shift schedule and determine what causes his perpetual lateness. After making the discovery, Mike and his manager can look into structuring a work schedule that will be more ideal for Mike and, at the same time, not cause any staff shortages in the course of the day.

Additionally, concerning the injured patient, the manager can arrange for a short training session where all members will get an opportunity to re-learn the importance of working in a safe environment, the consequences of an unsafe workplace, and what they can do to ensure each team member is involved in keeping the hospital safe for both staff and patients (Lach, 2010). The legal department will be welcomed in the training session to paint a clear picture of the cost of in-hospital patient injury on the staff that propagates it and the hospital.
