Case Study 9

Question 1: What is the phenomenon of concern to the client?

The main concern is that the Brown family is facing the challenge of making the final decision about moving their matriarch to the hospice in-patient facility, an option that is not in line with her wish to die at home surrounded by her family. The family is torn between admitting her into the in-patient hospice facility to elongate her lifespan or follow her will and letting her die in their hands at home.
Question 2: How are the concepts of person, health, environment, and nursing viewed in this situation using King’s theory?

King’s theory defines nursing as an interrelationship and interaction of a person with their environment or surroundings to achieve health and promote wellbeing. Nursing is considered a process, and the ultimate objective is to achieve health (Smith & Parker, 2015). Therefore, in this situation, nursing care is viewed as the process of providing hospice care, and the environment is the surrounding (including the relatives of the matriarch). On the other hand, the objective is to attain health (getting her from her coma).

Question 3: Describe strategies that would assist the family in attaining their mother’s goal.

There are several strategies the family can use to achieve their mother’s goal. Firstly, they can use the Goal Attainment Theory to assist their mother in regaining and maintain good health. This theory uses the concepts of communication, perception, self, interaction, growth & development, transaction, role, personal space, stress, and time. This is what King considers as the ‘Personal System,’ and also includes things like personal space as well as learning and coping. For example, family members can use coping and learning strategies advocated by Li & Xu (2012) to manage their anxiety, which can indirectly assist in the recovery of their mother.
Question 4: Describe how you would evaluate attaining this mutually agreed-upon goal?

I would evaluate the attainment or achievement of these mutually agreed-upon goals by assessing the outcomes of the hospice nursing interventions-the health and recovery of the mother. The goals can only be considered successful if the palliative hospice nursing care manages to address the spiritual and emotional needs of the matriarch.

Case Study 10

Question 1: How would the use of the Roy adaptation model assist the nurse in planning the continuation of care for Sergeant Johns?

The use of the Roy Adaptation Model (RAM), which was proposed by Sister Callista Roy in 1976, could assist in arranging the continuance of care for Sergeant Johns in three ways. Firstly, RAM can assist in the assessment of the way Sgt Johns is adapting and responding to his conditions, including traumatic brain surgery, change in social life, inability to work, and reduced mobility (Smith & Parker, 2015). The RAM model will also help nurses to design an effective nursing diagnosis plan as well as set goals pertinent to his conditions. Finally, the model can assist in the development of the intervention plans for the patient as well as establish the evaluation plan.
Question 2: Describe the influence of Roy’s theory in guiding the nurse’s actions in promoting Sergeant John’s adjusted self-concept.

The Roy Adaptation Model can guide the nurse’s actions in encouraging John’s adjusted self-concept in several ways. Most importantly, the nurse will use the model to assess the patient’s spiritual and physical self, as well as the body image or personal self, especially after losing his job and then suffering traumatic brain surgery.
Question 3: From the perspective of the Roy adaptation model, why is it important for the nurse to listen to Sgt. Johns’ “story” in his own words and not just base her interactions on information from the chart, fellow colleagues, or his family?

Based on the Roy Adaptation Model, it is paramount that the nurse listens to Sergeant John’s own story because this information might provide critical diagnosis clues. The nurse might be able to gather important clues regarding his perceptions, feelings, ideas, and values. The nurse might use this information to tailor specific interventions for the patient.

Case Study 11

Question 1: What are the effects of short-term loss of sleep and misuse of Ritalin on Eugenia’s normal state of wellness?

The misuse of Ritalin and the short-term loss of sleep have impacted Eugenia’s normal functioning and well-being in several ways. It has caused dizziness, shakiness, nausea, and a persistent headache. Besides, the two things have increased Eugenia’s blood pressure, level of anxiety, and heart rate (100 bpm). She has also lost her appetite because of excessive anxiety.
Question 2: What are the internal and external environmental stressors that have an influencing effect on Eugenia’s behavior and wellness?

The main internal stressor (originating from the psychological mindset) affecting Eugenia’s wellness and behavior is the fear of failing her chemistry paper. The external stressors, on the other hand, include failing to attend college and losing her car if she does not perform well. Another external stressor is the fear that her school might punish her for abusing a prescription drug (Ritalin), considering it is a Christian institution.
Question 3: Use Neuman’s concepts of primary, secondary, and tertiary prevention as an intervention in developing a plan of care for Eugenia

In developing Eugenia’s plan of care, the primary prevention interventions should focus on measures that should prevent the occurrence of internal and external stressors, especially the fear of failing the chemistry paper. This includes advising and encouraging Eugenia to prepare early enough for the chemistry paper and avoid the last-minute rush. On the other hand, secondary prevention requires detecting Eugenia’s symptoms, including fear, anxiety, blood pressure, heart rate, and loss of appetite, early enough and designing ways to prevent them. For example, the nurse could have detected the changes in her condition due to misuse of the drug during the first day rather than letting it spill to the second and third days. Finally, tertiary prevention concentrates on addressing Eugene’s current situation. This includes treating the symptoms, restoring her normal functions, and limiting her disability.
