Discussion – Ethics and Professionalism

Personal Philosophy

Nursing, to me, is more than just providing a treatment option to a patient. It is more focused on treating the patient first as a human being and as one who deserves to have his/her rights respected and upheld. With that in mind, it means delivering quality care to the person that is most suitable to him/her. It means going the extra mile to ensure that the person I offer my care and services to is satisfied with the process and, more so, with the outcomes.

Scope of APRN in Alabama

Alabama is considered among the more restrictive states when it comes to APRNs. Reimbursement of this category of healthcare practitioners remains at a very low level, which is currently at 70% of the physician reimbursement offering similar services. The many rural areas are unattractive for physicians; hence, the demand for APRNs is high, yet Alabama has some barriers to prevent the practice in the said areas. One of the barricades is the requirement to have a collaborative practice agreement between an APRN and a physician. The physician is required to be in the location of practice along with the nurse practitioner. However, this requirement is minimized to 25% contact after the APRN has worked in collaboration with a physician for two years. Additionally, a physician is restricted to a maximum of 4 APRNs working full-time. These restrictions hinder the expansion of much-needed healthcare, especially in rural Alabama (Quinney, 2014). Additionally, an APRN is required to earn 6 hours of continuing education (after approval) in pharmacology for every biennial renewal (Alabama Board of Nursing, n.d.).

The type of practice agreement needed is a written collaborative agreement between the APRN and a practicing physician. The two main questions are what happens if one fails the continuous education exams? Can an APRN automatically practice in another state? The resources in Alabama are few, especially in rural Alabama. The main resource that works to the advantage of healthcare provision is telemedicine. This allows for persons in remote areas to access care efficiently.
