5.    The Research Problem
5a:
While migraines are known to cause chronic pain, excessive medication usage, and medication overuse-related complications, there is limited knowledge and understanding of the analgesic medication usage behaviors of patients with migraines. People with migraines constantly use large amounts of analgesic medication for chronic pain. This results in analgesic medication overuse and its related complications, particularly medication overuse headaches (MOH). MOH further increases the need for analgesics, creating a vicious cycle of uncontrolled headaches and medication overuse. The deficit in the knowledge of the specific medication usage behaviors that encourage this cycle results in mismanagement and poor follow-up of patients with migraines by health practitioners. Therefore, this research seeks to fill this knowledge gap by investigating and characterizing the medication usage behaviors of these patients, such as the frequency, duration, and severity of attacks, the preferred choices of analgesic medication, frequency and criteria for medication usage, and the outcomes of medication use.
5b:
“This research which was descriptive and cross-sectional, was done with the aim of determining analgesic usage behaviors of patients with migraine” (Ozkan, Yilmaz, Kumsar, 2018). Found on page 1779.

“This study was done based on thoughts mentioned above and deficiencies in this field with the aim of guiding initiative programs which can be developed in order to assess analgesic usage of patients with migraine, provide healthcare personnel’s to review awareness of patients with migraine about drug management, prevent MOH which can develop due to analgesics overuse” (Ozkan, Yilmaz, Kumsar, 2018). Found on page 1779.

5c:
Although migraine is a common chronic and debilitating condition that causes serious socio-occupational dysfunctions during the most productive years of the lives of affected individuals, it is often misdiagnosed, mismanaged, and poorly followed up. Poor management of the condition predisposes the victims to cycles of chronic pain, medication overuse, medication overuse headaches and derails their quality of life.
5d:
Type of variable Variables in the Study Rationale
Independent variable Disease Characteristics of Patients with Migraine such as MIDAS stage, pain level according to VAS, frequency of attacks over the last three months, need for medicine, and general health status. Disease characteristics such as severity based on the MIDAs scale have a direct effect on the medication usage behaviors of patients with migraines. Changes in the disease characteristics, such as an increase in the frequency of attacks of headache, result in changes in medication use behaviors, such as more frequent use of analgesics or usage of stronger analgesics.
Dependent variable Analgesics Usage Behaviors of Patients with Migraine such as the choices of analgesic medication and reasons, the sources of analgesic medication, frequency of usage, analgesic overuse, the timing of usage and storage areas for medication.

Analgesic usage behaviors of patients with migraines are the variables being investigated and measured in this study. The various analgesic usage behaviors of participants in this study depend on and change based on the characteristics of their disease (migraine). For example, the choice of analgesic medication and the frequency of usage depends on the severity of the pain, and the frequency of attacks.
5e:
Operational definition Variable/Phenomenon of Interest Researcher’s definition Understanding as readers
Operational definitions Level of Migraine Disability

Level of disability due to migraine was defined as the stage of the patient on the MIDAS disability grading system (I-IV), with stage 4 indicating severe disability (headache for 30-90 days in the last three months) and pain level based on the visual analog scale of 0-10, with 0 being no pain, 10 being intense pain. of 9-10) (page 1779-1780). This definition implies that the degree of disability due to a migraine is determined based on the cumulative sum of the frequency of attacks, the intensity of pain, and the negative socio-occupational impacts.

Analgesic medication overuse

Analgesic medication overuse was defined as the use of analgesic medication for more than 15 days in a month in the last 3 months (page 1780). This definition implies that analgesic overuse means that a patient with a migraine uses analgesics for a duration equal to the duration when they are medication free.
Conceptual definitions Level of Migraine Disability The severity of the migraine-related disability is determined by the degree of disruption in the patient’s daily activities, work, family, and social life (Page 1778)

This definition means that severe disease in patients with migraines causes enough restraint to result in the subjective feeling of socio-occupational compromise.

6.    Review of Literature
6a:
The review of the literature puts the current study in context. The data on the burden and prevalence of migraine-related medication overuse (analgesics and migraine drugs are used excessively by 4% of the European, North American, and Asian populations, and the prevalence of MOH in tertiary care health centers is 30%-50%). (Ozkan, Yilmaz, Kumsar, 2018), Indicates the magnitude of the negative impact of inadequate knowledge of migraine-related medication usage behaviors and the subsequent mismanagement of the disease by health practitioners. The review also describes how the potential knowledge of analgesic medication usage behaviors of migraine patients can be applied in medication management by describing the known individual and collaborative roles of practitioners such as nurses, pharmacists, and doctors (Ozkan, Yilmaz, Kumsar, 2018). The association clearly indicates how the inadequate understanding of medication use behaviors limits the ability of health practitioners to manage migraine and migraine medication usage, resulting in a high prevalence of medication overuse, MOH, and disabilities.

The first three words of the last paragraph of the literature review are “It is a fact,” and it ends on page 1779.

6b:
The literature review of this study was based on data obtained from primary sources. The research report and data, such as the problem statements, the study methodology, and the findings in the Colas et al. (2004) and Diener & Limmroth (2004) studies showed that they were empirical studies.

6d:
There was no specifically identifiable heading for the literature review section. The sources cited in the section ranged from the year 2004 to 2017 with most of them published between 2011 and 2017. The most recent source is from the year 2017. The literature review section is on page 1779 of the report.
6e:
Disease management for people with migraine is largely patient dependent with a high rate of self-medication, low practitioner involvement. Current management strategies do not take the medication usage behaviors of patients into account. Analgesic medication overuse among patients with migraine can paradoxically worsen symptoms. Medication overuse and MOH are prevalent, and the burden continues to rise.

However, the analgesic medication usage behaviors of patients with migraine are poorly understood. The specific nature of analgesic medication overuse is uncharacterized. Besides, the most commonly used analgesics among Migraine patients are not known.

6f:
People suffering from migraine need to be diagnosed and treated by medical professionals. Additionally, a multidisciplinary approach is necessary for the management of migraine. Doctors, nurses, pharmacists, and patients should work collaboratively to manage migraines and prevent medication overuse and MOH. The literature review ended on page 1779.
